Sports Clearance Process: FinalForms
We are excited to announce that the Olympia School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season to season and year to year, meaning you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time. Visit your students school sports clearance page linked below for step by step directions.

Physical examinations are required for all interscholastic sports participants, including the activities of drill/dance/cheer squads. Verification of a physical examination must be submitted, at the time of clearance, on a form provided by the District and signed by a medical professional licensed to perform physical examinations. Certification for participation will be valid for a 13-month period from the date of the examination.

Transportation Forms
At times, school district transportation may not be available. In these cases, parents may choose to allow their child to drive or a parent can drive.
- Volunteer Driver Acknowledgement Form (English) (Spanish) (To be completed if a parent or student will be a volunteer driver for students).
- Athletic Event Transportation Passenger Form (English) (Spanish) To be completed and signed by parents of students who will be transported by a volunteer driver)
To be considered as a volunteer driver of students, this form must be completed and accepted by the school each season.

Students who access Home-Based Instruction, Running Start at the community colleges or Alternative Education programs may find themselves on a different academic calendar from their public school counterparts for WIAA academic eligibility. The Washington Interscholastic Activities Association (WIAA) has provided the following forms for students applying for WIAA eligibility.
