Special Dietary Needs Request Form
- Special Dietary Needs Request forms are available at each school building and through the Food Service office. Forms should be sent or faxed to each school building’s office. The school nurse receives a copy and sends it to that school’s kitchen, and they will submit a copy to the Food Service office.
- Forms must be signed by a licensed medical authority and filled out completely.
- Once a Special Dietary Needs Request has been submitted, Nutrition Services will work
immediately to accommodate the student’s needs.
- Nutrition Services will work to accommodate the students’ needs once they are aware of
request, while waiting for the form from medical authority.
- If the Special Dietary Needs Request is filled out incorrectly building Nurse will contact the
parent to inform them of the errors and work to get them filled out correctly. In the meantime,
accommodations will be made.
- Accommodations will be made in most cases if it does not alter the National School Lunch or
Breakfast program.
- A special Dietary Needs Request will be sent to the kitchen leads of the respective schools for
the accommodation. A copy will also be kept on file in the Nutrition Service office along with the
Nurse’s office.
- Nutrition Service’s will update the Skyward system to alert cashier of student’s Special Dietary
Need.
- If a student is no longer in need of an accommodation, Nutrition Service’s will only cease
accommodation with a signed form from the student’s parent or medical authority
- During Emergency closure and while operating under SFSP (Summer Food Service Program) we will work to accommodate all requests made to food service personnel. Nutrition Service will keep a working document to outline dietary needs, at which location the meals will be picked up at and by whom. The nutrition department will tell staff where the meals should be sent and when.
