High School Athletic Clearance 

To become eligable for participation in high school athletics, students and parents/legal guardians must complete and sign the following forms and return them on the date specified by the high school for clearance. 

These required forms may be completed on-line and printed from this site.  All completed and  signed forms must be returned to the building athletic department during the scheduled building athletic clearance days. 

Required Forms

Athletic Registration Form:

Each year students and parents are required to provide information related to residence, insurance coverage and permission to participate.  The "Athletic Registration" forms must be complete and returned during the initial clearance period each school year.

Student/Athlete Contract:

This contract states that students participating in athletic/activities programs has read, understands and will abide by the Olympia School District Athletic Code for High School Students found in policy 3200.  Students along with parents/legal guardians must read, sign and return this contract during their initial clearance period of each school year.

Physical Exam:

Physical examinations are rquired for all interscholastic sports participants, including the activities of drill/dance/cheer squads.  Verfication of a physical examination must be submitted, at the time of clearance, on a form provided by the District and signed by a medical professional licensed to perform physical examinations.  Certification for participation will  be valid for a 13-month period from the date of the examination.

The Olympia School District has joined with local health care providers to offer complete athletic physical examinations --  including EKGs and baseline cognative assessments (a component of the head injury protocol) --  at no cost.  These physicals are available to all Olympia School District high school athletes on the 1st Saturday of June at Capital High School.  A schedule of examination times will be posted in May.

Medical Travel Card:

Injuries and illness may occur at practices or games when parents/guardians are not immediately available.  The "Medical Travel Card" provides school staff with information directly related to health conditions, parent/guardian notification process, along with choices of physicians and hospitals.  This form must be completed, signed and returned during the clearance period for each season.

Transportation Forms:

At times, school district transportation may not be available.  In these cases students may choose to drive and/or ride with other students.  For students to drive or ride with other students "Athletic Event Transportation" forms must be completed, signed and returned during the clearance period for each season.

Head Injury and Concussion Acknowledgement

Washington state law and District policy require all parents/guardians and athletes to read the information regarding the signs/symptoms and the serious consequences of head injuries and concussions.  After reading this important information, both athletes and parents/guardians are required to sign and return the Concussion Acknowledgment Form prior to the first day of participation.

Assumption of Risk and Release of Liability / Safety Guidelines:

The "Assumption of Risk and Release of Liability" from along with the sports specific "Safety Guidelines" must be completed, signed and returned during the clearance period for each season.
