Staff forms

• pdf forms can be filled out and printed with Adobe Acrobat Reader.
  You cannot save pdf forms unless you have the full version of Adobe Acrobat.
• doc forms can be opened, saved, printed with any version of MS Word greater than Office 97.
• docx forms require Office 2007 or 2008
